Many patients find well-fitted scleral lenses comfortable because the lens vaults over the sensitive cornea and lands on tissue over the white part of the eye. Comfort is not guaranteed, however. Lens design, ocular surface health, filling solution, handling, and changes during hours of wear all affect how a scleral lens feels.
A new scleral lens can feel unfamiliar at first, but it should not be painful. Persistent discomfort is useful clinical information, not something a patient should be expected to tolerate.
What should a scleral lens feel like?
Early awareness of a larger lens can occur while you learn insertion, removal, and blinking with it. A successful fit should become consistently wearable and should not cause sharp pain, significant pressure, or worsening redness. Comfort can also change during the day as the lens settles and the ocular surface responds to wear.
Research in people with keratoconus has found higher average comfort satisfaction with scleral lenses than with smaller corneal gas-permeable lenses. The same research also found that some scleral lens wearers still reported discomfort, foggy vision, halos, and handling difficulty. The useful answer is therefore not “scleral lenses are always comfortable,” but “many patients can achieve good comfort when the lens, eye, and care routine are evaluated together.”
Why can a larger lens feel comfortable?
A scleral lens distributes its weight over a wider landing area and does not bear directly on the central cornea. It also holds a fluid reservoir over the corneal surface. For some patients with an irregular or sensitive cornea, this design is more stable and less irritating than a smaller lens that moves with each blink.
The fluid reservoir can be especially relevant for selected patients with dry eye or Sjögren’s syndrome and ocular surface disease. It may protect the surface during wear, but it does not correct every cause of dryness or eliminate the need for other treatment.
Seven factors that influence scleral lens comfort
- Landing-zone alignment: Uneven pressure, localized blanching, edge lift, or impingement can cause awareness or redness.
- Corneal and limbal clearance: The lens must vault the cornea and limbus appropriately after it settles.
- Lens centration and stability: Decentration can affect both comfort and optics.
- Ocular surface and eyelid health: Dryness, inflammation, allergy, mucus, and eyelid disease can change the wearing experience.
- Surface wetting: Deposits or poor wetting can create blur, dryness, and lens awareness.
- Insertion technique: An air bubble, trapped debris, or an incompletely filled lens can cause immediate discomfort or reduced vision.
- Solutions and care products: The filling, cleaning, and disinfection products must be appropriate for the lens and patient. Preserved products under the lens can irritate some eyes.
Comfort, fogging, and blur are different problems
Discomfort is a physical sensation such as pressure, burning, scratching, or pain. Midday fogging is a cloudy or hazy view caused by material accumulating in the fluid reservoir. Front-surface blur can result when the outside of the lens does not wet evenly or collects deposits.
These problems can occur together, but they do not always have the same cause. Removing and refilling a lens may temporarily clear fogging, while a pressure problem may require a design change. A fitting evaluation is more useful than trying random products or extending wear through discomfort.
Is there an adaptation period?
Some people adapt quickly, while others need more training and gradual experience. There is no single correct timeline. The learning curve often involves coordinating the eyelids, filling the lens without bubbles, centering it during insertion, and removing it safely.
Follow the wearing schedule provided by Dr. Boshnick. Do not lengthen wear time simply because the lens feels acceptable early in the day. Follow-up after real-world wear helps show how the lens settles and how the eye responds over time.
When should you remove the lens and call?
Remove the lens and contact the office promptly if you develop pain, increasing redness, light sensitivity, discharge, sudden or marked vision change, a lens that is unusually difficult to remove, or symptoms that worsen after insertion. Severe pain or sudden vision loss requires urgent eye care.
Do not sleep, shower, or swim in scleral lenses unless your treating doctor has given specific instructions that say otherwise. Never use tap water to fill, rinse, or store a contact lens. Use only the care system and preservative-free filling solution recommended for you.

- Keep follow-up visits even when the lenses feel comfortable.
- Report a recurring pattern: when symptoms begin, which eye is affected, and whether removal resolves them.
Frequently asked questions
Do scleral lenses hurt when they are first inserted?
They may feel unfamiliar, but they should not cause pain. Immediate sharp discomfort can indicate a bubble, debris, a surface problem, or a fit issue and should be addressed.
Are scleral lenses more comfortable than soft lenses?
It depends on the eye. A healthy regular cornea may do very well in a soft lens. A patient with an irregular or fragile cornea may find a scleral lens more stable and comfortable. The best comparison is made during a specialty evaluation.
Why does my scleral lens become cloudy after a few hours?
Midday fogging can be related to tear debris, inflammation, lens alignment, or other factors. Recurrent fogging should be evaluated because the correct response depends on the cause.
How many hours can I wear scleral lenses?
There is no universal safe number for every patient. Wear time should be individualized according to diagnosis, lens fit, eye health, and response during follow-up.
