Scleral lenses for pellucid marginal degeneration can improve vision by masking the irregular corneal optics that glasses and many soft lenses cannot fully correct. They do not reverse corneal thinning, stop progression, or replace medical monitoring. The result depends on the eye’s shape, health, visual potential, and a fit that remains safe after the lens settles.
Pellucid marginal degeneration, often shortened to PMD, can create large and irregular amounts of astigmatism. Patients may see ghost images, smearing, glare, or frequent prescription changes even while the cornea looks relatively clear. A specialty evaluation separates optical rehabilitation from treatment decisions intended to protect the cornea.
What is pellucid marginal degeneration?
PMD is an uncommon corneal ectasia. It is classically associated with a clear, crescent-shaped area of thinning near the lower edge of the cornea and protrusion just above that thin zone. The altered shape can produce high, irregular astigmatism and reduced visual quality.
PMD is often bilateral but may affect the two eyes differently. It can resemble keratoconus on a curvature map, and a “crab-claw” or “kissing-doves” pattern is not enough by itself to establish the diagnosis. Slit-lamp findings, corneal thickness measurements, topography or tomography, history, and change over time must be interpreted together.
Why might glasses or soft contacts stop providing clear vision?
Glasses correct regular refractive error well, but PMD can bend light in a more complex way. Higher-order aberrations and irregular astigmatism may leave overlapping letters, shadow images, halos, reduced contrast, or blur that changes across the pupil. A soft lens generally follows the uneven corneal surface, so its optics may not neutralize enough of that distortion.
Mild cases can still do well with glasses or another contact-lens design. Needing a scleral lens is not determined by the diagnosis alone; it depends on visual demands, correction achieved, comfort, corneal findings, and the patient’s ability to handle and care for a lens.
How do scleral lenses improve vision in PMD?
A scleral lens vaults over the cornea and lands on the sclera. Before application, its bowl is filled with sterile, preservative-free solution. The fluid reservoir and rigid front surface create a smoother optical system, reducing the effect of irregularity on the front of the cornea.
This design can also be more stable than a small corneal gas-permeable lens on selected PMD eyes, where inferior steepening can cause decentration or lens displacement. What does the research show?
Clinical studies and case series report improved corrected vision in many selected PMD eyes fitted with scleral lenses. A 2024 study of 24 eyes also reported improvement in contrast sensitivity and reductions in several measured higher-order aberrations. Earlier PMD series found meaningful visual gains for many patients who had not succeeded with other corrections.
Those findings are encouraging, but they do not mean every patient will obtain the same result. The published evidence includes relatively small, selected groups. Retinal disease, cataract, corneal scarring, amblyopia, optic-nerve disease, or internal optical aberrations can limit vision even when the front corneal surface is well corrected.
Why can scleral lens fitting be challenging in PMD?
The lowest and most protruding parts of a PMD cornea may be displaced from the center. Meanwhile, the sclera where the lens lands is not perfectly round. A lens must account for both surfaces while remaining centered enough to deliver stable optics.
The fitting process assesses:
- complete corneal and limbal vault after settling;
- avoidance of central, inferior, or limbal touch;
- a fluid reservoir that is sufficient without being unnecessarily deep;
- even distribution of landing-zone pressure;
- centration, movement, edge alignment, and removal;
- oxygen needs, material, thickness, and planned wearing time; and
- front-surface optics for residual astigmatism or aberrations.
A lens that looks acceptable immediately after application can change as it settles. Final judgment therefore requires observation after wear, not just an initial trial-lens view.
What happens during a PMD scleral lens evaluation?

Testing may include refraction, slit-lamp examination, corneal imaging, diagnostic lenses, over-refraction, and repeat assessment after settling.
Computerized corneal topography helps describe surface curvature, while tomography and pachymetry may provide additional information about elevation and thickness. These measurements support diagnosis and comparison, but no single map or number decides candidacy. For highly asymmetric ocular surfaces, an impression-based custom lens design may be considered when clinically appropriate; it is not required for every patient.
Can scleral lenses stop PMD from progressing?
No. Scleral lenses are used for optical rehabilitation and, in some cases, comfort during wear. They do not strengthen the cornea or prove that it is stable. PMD may progress slowly, and clear lens vision can coexist with a changing corneal shape.
Patients still need medical monitoring for corneal ectasia. If progression is suspected, a corneal specialist determines whether observation, cross-linking, or another intervention is appropriate. Candidacy and expected benefit cannot be assumed from approaches used for keratoconus.
Why do timing and monitoring of corneal maps matter?
Scleral lens wear can temporarily affect corneal thickness or curvature measurements. A 2024 short-term PMD study found small changes after six hours of wear and cautioned that they could complicate interpretation of progression. The clinician should know when lenses were last worn and may specify a lens-removal interval before imaging.
Monitoring may compare refraction, acuity, topography or tomography, pachymetry, slit-lamp findings, lens vault, landing-zone alignment, and the ocular-surface response. Bringing prior maps from the same instrument can make trends easier to interpret. Follow the schedule set by the clinicians managing the cornea and lens; good comfort is not a substitute for those examinations.
What should patients expect from daily wear and care?
Successful wear requires consistent application, removal, cleaning, disinfection, and case replacement. Fill the lens only with the sterile, preservative-free solution recommended for the lens reservoir. Do not use tap water, homemade saline, saliva, or a preserved product unless the treating professional has specifically instructed that use.
Air bubbles can reduce vision and create localized dryness, while deposits, poor surface wetting, or reservoir debris can cause haze. A lens that becomes increasingly red, painful, cloudy, tight, or difficult to remove should not simply be worn longer. Remove it as instructed and contact the fitting office.
When do symptoms require urgent care?
Remove the lens and seek prompt professional advice for sudden vision loss, new or increasing pain, marked redness, light sensitivity, discharge, a white spot on the cornea, injury, or a lens that cannot be removed safely. Sudden severe pain and haze can rarely accompany acute corneal hydrops in ectatic disease and require urgent examination.
Do not sleep in a scleral lens or continue wearing it through an acute eye problem unless a treating clinician has given explicit medical instructions. If symptoms are severe or the office is unavailable, seek urgent eye care or emergency evaluation.
Frequently asked questions
Are PMD and keratoconus the same condition?
No. Both are ectatic disorders and can produce irregular astigmatism, but their typical thinning patterns differ. They can also overlap or be confused on maps, so diagnosis should integrate examination, thickness data, and serial imaging.
Will scleral lenses make my cornea thinner?
Scleral lenses are not intended to thin the cornea. However, fit, reservoir depth, oxygen delivery, and wear time can affect corneal physiology. Regular examination is needed, especially in an already thin or compromised cornea.
Can both eyes use the same scleral lens?
Usually not. PMD is commonly asymmetric, and each eye needs its own measurements, fit, and prescription. Lenses should be clearly identified and never interchanged unless the fitting professional confirms that plan.
Will I still need corneal imaging if my vision is good?
Yes. Vision through a lens shows how well the optics work while it is worn; it does not show whether PMD is stable. Serial examination and imaging answer a different, medically important question.
