Depending on the stage and cause, medical care may include lubrication, medication changes, serum-based drops, therapeutic agents, protective procedures, or surgery. Why is fitting more complex in neurotrophic keratitis?

What are the limitations and risks?

Scleral lenses require careful hygiene and follow-up. Potential problems include infection, deposits, solution sensitivity, corneal swelling, fogging, redness, handling trauma, and pressure from an unsuitable fit. Reduced sensation can delay a patient’s awareness of some complications.

The lens should be removed and the treating office contacted promptly for sudden blur, increasing redness, light sensitivity, discharge, a white corneal spot, or a change in surface appearance. The absence of pain does not make these findings unimportant.

Can scleral lenses improve vision in neurotrophic keratitis?

They may improve vision when an irregular or damaged anterior corneal surface is a major source of optical distortion. The rigid front surface and fluid reservoir can create a smoother optical system. Visual improvement may be limited when scarring is dense or when cataract, retinal disease, optic-nerve damage, or another condition is present.

A diagnostic lens trial helps estimate how much of the reduced vision is related to the corneal surface. It does not guarantee the result of a finalized custom lens.

How is this different from ordinary dry-eye fitting?

Neurotrophic keratitis involves impaired corneal innervation and a risk of epithelial breakdown. A patient may also have dry eye, but the reduced sensation and healing problem change the level of medical concern and monitoring required.
