Scleral lenses may improve vision and support the corneal surface in selected patients with limbal stem cell deficiency, but they do not replace missing stem cells or cure the condition. The fit must protect the limbal region, maintain appropriate clearance, and be checked regularly because a compromised ocular surface may respond differently over time.
Limbal stem cell deficiency, often shortened to LSCD, ranges from localized disease to extensive failure of the corneal surface. A scleral lens can be valuable within a coordinated plan, yet its benefit and safety depend on the cause and stage of LSCD, the shape of the eye, the condition of the eyelids and tear film, and the patient’s ability to handle and care for the lens.
What is limbal stem cell deficiency?
The limbus is the transition zone between the clear cornea and the white of the eye. It contains stem and progenitor cells that help renew the corneal epithelium, the thin protective layer on the front of the cornea. In LSCD, too few healthy cells remain, or their supporting environment no longer works normally.
The surface may then heal poorly or become covered by conjunctival-type tissue. LSCD may follow chemical or thermal injury, repeated surgery, chronic inflammation, infection, medication toxicity, long-term contact lens stress, or an inherited condition. Identifying the cause matters because it affects both medical treatment and lens planning.
What symptoms and signs can LSCD cause?
Possible symptoms include fluctuating or reduced vision, redness, burning, foreign-body sensation, tearing, dryness, and light sensitivity. The examination may show irregular epithelium, late fluorescein staining, blood-vessel growth, persistent or recurrent epithelial defects, scarring, or conjunctival tissue extending onto the cornea.
Those findings are not unique to LSCD. Diagnosis may require a cornea specialist’s examination and, when available, tests such as impression cytology, in-vivo confocal microscopy, or anterior-segment imaging. A symptom list or photograph cannot establish the diagnosis or stage.
How may scleral lenses help limbal stem cell deficiency?
A scleral lens is a large, gas-permeable lens designed to vault the cornea and limbus and land on conjunctival tissue over the sclera. Its bowl is filled with sterile, preservative-free solution before application. This creates a fluid reservoir over the corneal surface during wear.
For an appropriately selected eye, the reservoir and rigid front surface may reduce eyelid friction, shield fragile epithelium during wear, and create smoother optics when surface irregularity affects vision. Published retrospective series have reported visual improvement and relative surface stability for many fitted eyes, but individual responses vary and the available studies are not guarantees of outcome.
What can a scleral lens not do?
A scleral lens does not create new limbal stem cells, reverse established scarring, or treat the cause of autoimmune or inflammatory disease. It also cannot correct vision loss arising from the retina, optic nerve, advanced cataract, or dense corneal opacity.
The lens is best understood as a rehabilitation and surface-support tool. Stem-cell transplantation, inflammation control, eyelid treatment, lubrication, or other medical and surgical care may still be necessary. Clearer vision through a diagnostic lens does not prove that LSCD is stable.
Who may be evaluated for scleral lens wear?
An evaluation may be considered when LSCD causes an irregular optical surface, recurrent surface breakdown, significant symptoms, or limited function despite other care. Candidacy is not based on diagnosis alone. Active infection, an open epithelial defect, severe inflammation, very fragile tissue, poor lens-handling ability, or anatomy that prevents safe landing may change the plan.
Patients with other forms of corneal disease may have similar complaints but different risks. Current records, prior procedures, medications, and the treating cornea specialist’s recommendations help define the appropriate role of a lens.
Why must the fit protect the limbus?
The limbus is already vulnerable in LSCD. The lens should maintain appropriate corneal and limbal clearance after settling while avoiding focal pressure, suction, or tissue impingement. Excessive fluid depth, poor tear exchange, lens decentration, and a tight landing zone can also influence oxygen delivery and surface response.
A study of 27 LSCD eyes found that disease stage worsened in some eyes during scleral-lens use. The investigators could not prove that the lens caused the change, but imaging in a subset showed limbal compression and delayed filling, raising concern about hypoxia. The practical lesson is careful customization and follow-up, not that every person with LSCD should avoid scleral lenses.

What monitoring is needed?
Monitoring frequency depends on LSCD severity, disease activity, wear time, epithelial health, and any concurrent treatment. Early follow-up may be closer than for an uncomplicated vision-correction fit. Visits may assess corneal staining, epithelial defects, new blood vessels, limbal clearance, conjunctival compression, corneal swelling, lens deposits, reservoir debris, redness after removal, and changes in vision.
Visual acuity alone is not a sufficient safety test. One LSCD study found that vision could remain useful even when the ocular-surface stage changed. Patients should keep scheduled medical and lens visits even when the lens feels comfortable. The same principle applies to other complex ocular-surface conditions.
Will other treatment still be needed?
Often, yes. The medical plan may address inflammation, dryness, eyelid problems, medication toxicity, infection risk, or the original injury. More advanced disease may require procedures intended to restore or reconstruct the surface. These decisions belong to the professionals managing the underlying condition.
Do not stop prescribed drops or systemic medicines because the eye feels better with a lens. A scleral lens and medical therapy can address different problems, and improvement during wear does not show that the cellular disease has resolved.
When should a patient seek urgent eye care?
Remove the lens and contact the treating eye-care team promptly for sudden or substantial vision loss, increasing redness, new or worsening pain, marked light sensitivity, discharge, a white spot on the cornea, an eye injury, or a suspected epithelial defect. Seek emergency eye care if prompt advice is unavailable.
Do not resume wear through an acute problem unless the clinician examining the eye specifically directs it. LSCD can reduce the surface’s ability to recover, so waiting for a routine appointment may be unsafe.
