A scleral lens may protect the corneal surface, reduce dryness and mechanical friction during wear, and improve functional vision in selected patients with ocular cicatricial pemphigoid. It is an ancillary rehabilitation tool, not a treatment for the autoimmune inflammation that causes progressive conjunctival scarring.
Ocular cicatricial pemphigoid, often abbreviated OCP, is now commonly described as ocular mucous membrane pemphigoid, or ocular MMP. It can threaten vision and may affect other mucous membranes. Prompt diagnosis, systemic disease control, careful lens fitting, and continuing medical follow-up all matter. Feeling better with a lens does not mean the disease is inactive.
What is ocular cicatricial pemphigoid?
Ocular MMP is a chronic autoimmune disease in which inflammation affects the conjunctiva, the membrane covering the white of the eye and lining the eyelids. Repeated inflammation can lead to fibrosis and progressive scarring. The disease may also involve the mouth, nose, throat, skin, or other mucous membranes, although the pattern differs from person to person.
Diagnosis can be difficult because early findings may resemble dry eye, medication toxicity, allergy, or another chronic conjunctivitis. A specialist considers the history, examination pattern, scarring, and laboratory or biopsy findings when available. A negative biopsy does not always settle the question, so results must be interpreted in clinical context.
What symptoms and complications can occur?
Patients may report redness, burning, grittiness, tearing, dryness, light sensitivity, or blurred vision. Over time, scarring can shorten the conjunctival fornices, form adhesions between the eyelid and eye called symblepharon, rotate the eyelid inward, or direct lashes toward the cornea. Tear and eyelid function may deteriorate.
The cornea can then develop staining, recurrent or persistent epithelial defects, blood-vessel growth, infection, thinning, or scarring. Ordinary dry-eye care may ease symptoms but does not address the underlying autoimmune process. Symptoms alone are also an unreliable measure of whether scarring is progressing.
Why is systemic medical treatment essential?
Ocular MMP is considered a high-risk form of mucous membrane pemphigoid because uncontrolled disease can permanently damage the ocular surface. Treatment commonly requires systemic immunomodulatory therapy selected and monitored by physicians with relevant expertise. The exact medicine depends on disease severity, health history, response, and risk.
Lubricants, topical anti-inflammatory medicines, eyelid procedures, and specialty lenses may manage surface consequences, but they do not reliably stop the circulating immune process by themselves. Do not stop or alter prescribed therapy because a scleral lens improves comfort. Medication changes require the clinician managing OCP and any associated systemic disease.
How may a scleral lens help OCP?
A scleral lens is a large, gas-permeable lens designed to vault the cornea and limbus. Its bowl is filled with sterile, preservative-free solution before application, creating a fluid reservoir over the cornea. For a suitable eye, that reservoir can reduce desiccation and shield the surface from some eyelid and lash friction during wear.
The rigid front surface may also provide clearer optics when an irregular tear film or corneal surface is degrading vision. In a 2025 retrospective study of 20 fitted OCP patients, all reported better comfort, mean keratopathy grade and visual acuity improved, and 96% of fitted eyes continued wear. These are encouraging results from a selected clinical cohort, not a promise that every patient will obtain the same response.
An earlier report described healing of persistent epithelial defects in one patient using mini-scleral lenses while other treatment continued. That case supports a possible protective role but cannot prove that a lens alone heals epithelial disease. Similar distinctions apply across severe ocular-surface conditions.
What can a scleral lens not do?
A scleral lens cannot switch off the autoimmune response, reverse established conjunctival fibrosis, correct eyelid malposition, remove misdirected lashes, or guarantee that the cornea will remain stable. Scleral lenses do not replace biopsy, systemic medication, cornea care, or evaluation of other mucous membranes.
Vision may remain limited by dense corneal scarring, glaucoma, cataract, retinal disease, optic-nerve damage, or another condition. A diagnostic lens trial can estimate how much blur comes from the front optical surface, but it cannot predict every aspect of long-term comfort, wear time, or disease behavior.
Why can scleral lens fitting be difficult in OCP?
Fornix shortening, symblepharon, conjunctival scarring, eyelid changes, fragile epithelium, trichiasis, and an irregular landing area can affect lens size, centration, movement, and pressure distribution. A lens edge that crosses a scar band or compressed area may become uncomfortable or create tissue stress even if initial vision is good.
The lens must clear the cornea and limbus after settling without unnecessary fluid depth, and its landing zone should avoid focal blanching or impingement. Some unusually complex eyes may be considered for an impression-based design such as EyePrintPRO. It is an option for selected anatomy, not an automatic upgrade or a guarantee of safe wear.

Records from the treating ophthalmologist or ocular-immunology team can help distinguish medical disease control from the goals of visual rehabilitation.
A diagnostic lens is assessed on the eye for clearance, landing, centration, comfort, and potential vision. Because a scleral lens settles during wear, the first few minutes are not the complete fitting result. Examination after a longer trial and after removal may reveal compression, redness, staining, or an edge interaction that requires redesign.
What daily care and wearing precautions matter?
Use only the prescribed lens cleaner and disinfection system, and fill the lens with the recommended sterile, preservative-free solution. Do not use tap water, saliva, homemade saline, or a previously opened nonsterile mixture. Wash and dry hands before handling, inspect the applied lens for bubbles, and follow the replacement schedule for the lens, case, and care products.
Do not add prescription drops to the lens reservoir unless the prescribing clinician gives explicit instructions. Routine sleeping in a scleral lens is not recommended. Any therapeutic extended-wear plan requires direct medical supervision because infection, oxygen, medication, and follow-up considerations differ from ordinary daytime wear.
How are the lens and autoimmune disease monitored?
Scleral-lens visits may assess vision, epithelial staining, fluid-reservoir quality, deposits, limbal clearance, landing-zone pressure, redness after removal, corneal swelling, and handling technique. Medical visits separately assess conjunctival inflammation, progressive fibrosis, eyelid and lash complications, corneal integrity, medication effectiveness, and systemic adverse effects.
Both schedules remain important. A comfortable lens can mask neither progressive scarring nor a medication complication. Coordination may involve a cornea or ocular-surface ophthalmologist and, depending on nonocular involvement and treatment, dermatology, rheumatology, oral medicine, or another medical specialist.
When should a patient seek urgent care?
Remove the lens and contact the treating eye-care team promptly for sudden or substantial vision loss, increasing redness, new or worsening pain, marked light sensitivity, discharge, a white corneal spot, injury, or a suspected epithelial defect. Seek emergency eye care if prompt specialist advice is unavailable.
New mouth sores or problems involving swallowing, breathing, the nose, skin, or another mucous membrane should also be reported promptly to the appropriate physician. Do not continue lens wear through an acute eye problem unless the clinician examining the eye specifically directs it.
Frequently asked questions
Does a scleral lens treat the autoimmune disease?
No. It may support the cornea and improve vision during wear, but systemic medical therapy is used to control the immune-driven inflammation. The lens and medication have different roles.
Can I wear a scleral lens during an OCP flare?
Only according to the treating team’s instructions. Active inflammation, an epithelial defect, infection concern, or changing anatomy may require a pause, a revised schedule, or a new fit while medical treatment is addressed.
Can a scleral lens protect the cornea from eyelashes?
It may reduce some mechanical contact during wear, but trichiasis and eyelid malposition still require evaluation. A lens does not permanently redirect lashes or correct an inward-turning eyelid.
What if conjunctival scarring makes insertion difficult?
Handling training, application aids, or a different lens diameter or geometry may help some patients. In advanced scarring, safe application or landing may remain impractical. The examination determines whether further customization is reasonable.
